Annual Membership Dues

  • First family member $299 ($850+ Value)

  • Additional family members $199 ($850+ Value)

  • Additional optional Periodontal plan $458 ($1500+ Value)

Note: All family members must reside in the same household. Dependent children must be 21 years or younger at the time of enrollment​

Terms and Limitations of the Plan

This dental discount plan is NOT dental insurance and cannot be combined with any other dental insurance plan. The plan is valid only at Five Oaks Dental. If you are referred to a specialist, the discount will not apply to treatment provided outside our office.

Services related to dental injuries covered by workers' compensation, disability insurance, legal settlements, or outside medical care are not eligible under this plan.

This plan is non-transferable and may not be shared with or transferred to another family member. It is also non-refundable. No refunds will be issued if a patient chooses not to use their membership benefits. Rates are subject to change annually.

Payment for services is due at the time of treatment. If you choose to finance treatment through CareCredit®, the discount may be adjusted to account for merchant processing fees. This offer cannot be combined with any other promotions or discounts.

For orthodontic treatment, participants must remain active members of the plan for the entire duration of treatment.

This plan covers dental services only. Dental products and retail items are not included.
